Dear Parents of Rising 7th graders: This is a reminder that ALL students born after Jan 1, 2002 who are attending 7th grade must have received one dose of Tdap vaccine and one dose of meningococcal vaccine. According to Georgia law, form 3231 must be provided to the school as documentation that your student is current on ALL vaccinations. All parents, please contact your healthcare provider to ensure your child is up to date so we can ensure there will be no disruption to their upcoming school year.
